Please click on one of the following Lutheran Via de Cristo of North Texas (LVDCNT) Registration Form links (PDF, Word) to download, complete and print the form on letter size paper.  After completing the entire form, including participant's, pastor's, and sponsor's signatures, email first, then snail-mail the form to:

After Tricia receives your registration she will check it for completeness, and contact you to answer any questions she may have and finalize the registration. She will then mail you a letter with detailed instructions about your responsibilities as a sponsor. Please read the entire letter and contact Tricia if you have any questions.
